How to get our services:
Requirements:
1. A person must be homebound. A person who is homebound only leaves
the home on short and infrequent periods and uses a taxing effort.
2. A person must have a “skilled need”. A skilled need
may be related to a new or exacerbated disease process, a new or
changed medication or a recent hospitalization.
3. A person must be under the care of a physician. This physician
must agree to order the home health services.
4. Payment is made through the person’s insurance, Medicare,
Medicaid, or Private insurance.
